Password: Last Name
Your Member Number must be entered as a six digit number. Add leading zeros until you have
a six digit number.
IE: If your Member Number is: 1234,
enter 001234 in the User Name Field.
Please note that if you are the spouse on record, you must append a "-1" to the member number. For example: 001234-1.
